This service is accessible to patients after seeing a physician that uses Chartscape's Electronic Medical Record system (EMR). The service is optional and the cost of the service is 30 dollars per year. Payment forms include Visa, MasterCard and American Express.

The medical record storage fee sale is final. There are no refunds, returns, or cancellation options.

All patient information collected using this service is stored in a completely secure and HIPAA compliant manner. Information entered by both the patient and the physician will be collected and stored within this service. This information will never be sold or used for marketing purposes. As part of this service, the information may be shared with HIPAA related vendors with both the doctor's consent as well as the patient approval.
